COMPREHENSIVE MEDICATION MANAGEMENT AND HEALTH COACHING

I work closely with you and your other healthcare providers to make sure your medications are working as effectively as possible and not causing any problems. I answer your questions, and provide helpful information about how to take your medications safely and get the most benefit from them. The goal is to help you stay healthy and feel your best while taking the fewest medications possible.

This package also includes twelve health coaching sessions. We will dig deep to find the root cause of your health problems and how to handle the struggles of your life.

Step 1- Book your first session with me

This is a complimentary meeting where we’ll focus on getting to know each other and discussing your health goals and concerns. In this conversation we will establish a collaborative relationship where you feel comfortable sharing your health journey with me. This session will be done over a Zoom Meeting, it will help you determine whether working together will benefit you and you will decide if you want to continue with the next step with me.

Step 2 – Book your second session

During this appointment, we will review your Health History Form, discuss your current and past medications, as well as the lab results that you have available; and will order other labs if needed.

Step 3 – Book your third session

We will discuss any medication related problems you may be having. It includes and is not limited to: interactions, duplications, side effects, etc. Based on the results, we will let your provider(s) know so that they can review your medications and monitor you more closely. Supplement recommendations will also be discussed as appropriate. Within one week of our appointment you will receive a document highlighting what we talked about. 

Step 4- Get ready to transform your life

If we realize that we are a good fit for each other, we will have 12 weekly sessions. In these sessions we will determine your nutritional and wellness needs as well as what changes in your diet or lifestyle will help you become an advocate of your own health and in turn build a happier, more productive, and more fulfilled life.

Please note, that as a pharmacist, I cannot legally diagnose or prescribe medications. If you need to make changes to your medications and/or dosage, it must be done by your medical provider.
